Job summary

4 YOUR PEACE OF MIND LLC is seeking a Connecticut-licensed Physical Therapist to provide comprehensive PT services in acute and home health settings. You will assess patient needs, develop care plans, and coordinate with physicians and care teams while educating patients and families on safe practices and equipment use.

The role requires ongoing documentation, supervision of assistants, and a focus on restoring function, mobility, and reducing pain.

Qualifications

• Licensed by the Connecticut Board of Physical Therapy Examiners.
• Ability to maintain confidentiality and communicate with stakeholders.
• Proficient in standard office equipment and software.
• Strong problem-solving and analytical thinking skills.

Responsibilities

• Provide initial and ongoing assessments including OASIS at appropriate time points.
• Develop and revise the plan of care in consultation with physician and care-team members.
• Plan, evaluate, and treat assigned caseload and document progress.
• Supervise therapy assistants and home health aides as appropriate.
• Educate patients and/or families about proper safe use and adaptive equipment.
