Job summary

CareCo Medical Home Health is seeking a licensed Physical Therapist to provide skilled in-home therapy services to patients in the community. This role collaborates with a supportive interdisciplinary team and supervises PT assistants while maintaining adherence to agency policies.

The position offers full-time, on-site work in Connecticut with mileage reimbursement between visits and flexible scheduling to balance patient care and drive time.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Connecticut.
• Minimum of three years of healthcare experience.
• Home health and OASIS experience preferred.
• Strong communication and documentation skills.
• Ability to work independently with minimal supervision.
• Valid driver's license and insured vehicle.

Responsibilities

• Perform physical therapy evaluations, assessments, and skilled treatments in the home.
• Complete initial and comprehensive assessments, including OASIS, within required timeframes.
• Develop, implement, and update individualized Plans of Care.
• Document all visits, findings, and outcomes in a timely manner.
• Communicate changes in patient status to physicians, case managers, and the care team.
• Provide patient and caregiver education to promote recovery and safety.
• Participate in discharge planning and interdisciplinary care coordination.
• Supervise Physical Therapy Assistants in accordance with agency and regulatory guidelines.
• Promote patient safety and comply with all agency, state, and federal standards.
