Job summary

CareCo Medical Home Health seeks a licensed Physical Therapist to provide skilled in-home therapy to patients in Connecticut. The role offers per diem/per visit compensation, with mileage reimbursement and flexible scheduling.

The clinician will evaluate and treat patients at home, complete Oasis-driven documentation, and collaborate with nurses and other disciplines under supervision. A CT license and reliable vehicle are required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice (Connecticut).
• Minimum of three years of healthcare experience.
• Home health and OASIS experience preferred.
• Strong communication and documentation skills.
• Ability to work independently with minimal supervision.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Perform PT evaluations, assessments, and in-home treatments.
• Complete initial and comprehensive assessments, including OASIS, within required timeframes.
• Develop, implement, and update individualized Plans of Care.
• Document all visits, findings, and outcomes promptly.
• Communicate changes in patient status to physicians and care team.
