Hartford HealthCare at Home is seeking a dedicated Physical Therapist to join our home health team in Mansfield, CT. This role emphasizes one-on-one patient care to restore strength, mobility, and independence in patients' homes.

The position offers flexible scheduling with options for full-time, part-time, or per diem/Pay Per Visit, and benefits eligible at 24+ hours per week, within Hartford HealthCare's statewide network.

Competenze

• Graduate of an accredited Physical Therapy program.
• Active Connecticut Physical Therapist License.
• CPR Certification required by start date.
• Prior Physical Therapy experience.

Mansioni

• Evaluate and treat homebound patients referred for physical therapy services.
• Develop individualized treatment plans designed to maximize patient independence and function.
• Assess strength, balance, mobility, endurance, gait, pain, and activities of daily living.
• Deliver evidence-based therapeutic interventions and exercises.
• Collaborate with physicians, nurses, and interdisciplinary care teams.
• Educate patients and caregivers to support recovery and long-term success.
• Monitor progress and adjust treatment plans as needed.
