Job summary

Avir at the French Home in San Antonio, TX is seeking a compassionate Physical Therapist to join our team on a per diem basis. You will evaluate residents' mobility, design individualized treatment plans, and provide therapeutic exercises to improve independence and quality of life.

Collaborate with nursing, physicians, and other therapists; educate residents and families on exercises and safety; and maintain accurate documentation while promoting safety and functional outcomes for older adults

Qualifications

• Current Physical Therapist license in the State of Texas.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification, as required.
• Strong clinical, communication, organizational, and interpersonal skills.
• Experience working with older adults or in a skilled nursing/rehabilitation setting is preferred.
• Ability to work independently while collaborating effectively with an interdisciplinary team.

Responsibilities

• Evaluate residents to identify physical limitations, mobility concerns, and functional needs.
• Develop individualized treatment plans based on each resident's condition and goals.
• Provide therapeutic exercises, mobility training, balance activities, gait training, and other appropriate interventions.
• Monitor resident progress and modify treatment plans as needed.
• Educate residents, families, and caregivers on exercises, mobility techniques, and safety.
• Collaborate with nursing staff, physicians, occupational therapists, speech therapists, and other members of the care team.
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements.
• Promote resident safety and independence during all therapy activities.
• Participate in care planning and communicate changes in resident status to the appropriate team members.
• Follow professional, ethical, and facility standards of practice.
