Job summary

Avir at the French Home in San Antonio, TX is seeking a compassionate Physical Therapist to join the team on a per diem basis. The PT will evaluate residents, design individualized plans, and guide exercises to improve mobility, strength, balance, and independence.

The role requires a Texas PT license, a DPT or equivalent degree, and CPR certification. Collaboration with a multidisciplinary team and thorough documentation are essential.

Qualifications

• Must hold a Texas Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification required.
• Strong clinical, communication, organizational, and interpersonal skills.
• Experience with older adults or skilled nursing/rehabilitation setting preferred.

Responsibilities

• Evaluate residents to identify physical limitations and mobility concerns.
• Develop individualized treatment plans based on goals.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Monitor progress and adjust plans as needed.
• Educate residents and families on exercises and safety.
• Collaborate with nursing, physicians, and other therapists.
• Maintain accurate clinical documentation per regulations.
• Promote resident safety and independence during therapy.
