Job summary

UnitedHealth Group's LHC Group is seeking a Senior Living Physical Therapist to deliver age-appropriate rehab for adults to geriatrics in outpatient and home settings. You will assess patients, develop and monitor treatment plans, supervise support staff, and educate patients and families while documenting care per facility standards.

The role emphasizes direct patient care, team conferencing, and adherence to safety protocols, with a pay range that reflects a competitive compensation package

Qualifications

• Current Physical Therapy licensure in state of practice
• Current CPR certification
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Responsibilities

• Evaluate the patient to determine rehab potential and set an individualized plan of care
• Set initial STG's and LTG's
• Utilize equipment in a proper and safe manner; maintain logs
• Maintains equipment logs for required equipment in the outpatient clinic
• Follow medical regimen and adhere to precautions (THR, TKR, Cardiac, Wound)
• Utilizes safe treatment techniques: transfers, positioning, splinting, paraffin bath, ultrasound, electrical stimulation
• Complete discharge summary on final outpatient PT visit
