Job summary

LHC Group, part of the Optum family, is seeking a Senior Living Physical Therapist to provide age-appropriate services to adults and geriatrics in home health settings. You will assess, treat, supervise PTA/tech, develop home exercise programs, and participate in team and family conferences.

Responsibilities include program development, documentation, adherence to department standards, and working with multidisciplinary teams. A strong patient-centered approach is required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient to determine rehab potential and sets goals in the team plan of care.
• Sets initial STG's and LTG's.
• Uses safe treatment techniques (transfers, positioning, splinting, ultrasound, etc.).
• Completes D/C summary on final visit to outpatient PT.
• Maintains equipment and follows medical regimens and precautions.
