Job summary

Optum Home & Community Care, part of the Optum family, is expanding its Senior Community Care program to provide high-quality clinical care to patients in nursing homes, senior housing and assisted living settings. We seek highly skilled Advanced Practice Clinicians to join our SCC team on a per-diem basis.

This role offers a $5,000 sign-on bonus for external candidates, a schedule of 24 hours a month you pick, and no on-call, weekend, or holiday requirements.

Qualifications

• Active ANCC or AANP certification for NP; NCCPA certification for PA.
• Active, unrestricted licensure in the practicing state with no disciplinary actions.
• Ability to obtain prescriptive authority and a collaborative practice agreement if required.

Responsibilities

• Conduct comprehensive assessments including medical history, vitals, and physical examination.
• Support short-term sub-acute rehabilitation episodes and care planning.
• Document accurately with ICD-10 and coordinate with the care team.
• Provide counseling and education to patients and caregivers.
• Navigate varied care environments to meet patient needs.
