OneWell Health Care in Pennsylvania seeks a Per Diem Physical Therapist to provide home and community-based care for individuals with IDD. You’ll evaluate mobility, develop goal-based plans, and collaborate with families and caregivers to improve independence.

This field-based role offers flexible scheduling, ongoing support, and opportunities to impact patients’ quality of life across settings. Applicants must have a PT degree and state license; CPR is preferred; travel is required; bilingual

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.
• Help individuals improve mobility, coordination, physical function, and independence.
• Recommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
