Job summary

FreedomCare LLC is seeking a Per-Diem Field Registered Nurse to join our Licensed Home Care team in Queens, NY. This field-based role requires frequent travel to patients’ homes and involves assessments, care planning, and caregiver supervision.

The ideal candidate holds a BSN or an associate degree in nursing with homecare experience, an active NYS license or compact license, and 1–2 years in geriatric/homecare.

Qualifications

• Bachelor’s Degree (BSN) or an associate’s degree in nursing with relevant homecare experience.
• Current NYS License or compact License in good standing.
• 1–2 years of nursing experience in homecare; geriatric or disability care preferred.
• Ability to travel within NYC boroughs; valid driver’s license and reliable transportation.

Responsibilities

• Conduct initial assessments to determine care needs and develop individualized care plans.
• Train and supervise non-skilled caregivers in personal care and daily living activities.
• Monitor client well‑being and report changes to healthcare professionals.
• Educate clients and families on care strategies, safety, and daily living skills.
• Ensure compliance with agency policies and state regulations; maintain confidentiality of records.
• Maintain accurate documentation of client care and progress; act as liaison among clients, families, and caregivers.
