Qualifications

• Must be a licensed driver with independent transportation and current insurance coverage.

Responsibilities

• Administers the appropriate Care Plan in collaboration with care team members.
• Offers ongoing instructions and care plan review to Caregivers during field visits.
• Creates Plans of Care tailored to meet the client’s individual needs.
