Job summary

FreedomCare in New York is seeking a Per-Diem Field RN for our Licensed Home Care team. This field-based role requires frequent travel to patients' homes in Brooklyn and Staten Island.

You will conduct initial assessments, develop care plans, train and supervise non-skilled caregivers, monitor client well‑being, and ensure compliance with state regulations and agency policies. A NYS RN license (or compact), BSN preferred, 1–2 years homecare experience, and a valid driver’s license are required.

Qualifications

• Bachelor’s degree in nursing and active NYS license or compact license in good standing.
• 1–2 years nursing experience with homecare exposure.
• Experience in geriatric care or with individuals with disabilities.
• Ability to travel within NYC boroughs.
• Proficient in Microsoft Office Suite and EHR systems.

Responsibilities

• Conduct initial client assessments to determine care needs and create individualized plans.
• Train and supervise non-skilled caregivers in personal care and daily living activities.
• Monitor clients’ well-being and report changes to healthcare professionals.
• Educate clients and families on care strategies, safety, and daily living skills.
• Ensure compliance with agency policies and state regulations; maintain confidential documentation.
