Job summary

People Care is seeking a Field Nurse to visit patients at home, assess their status, develop and manage the plan of care, and supervise home health aides. This salaried, exempt role requires frequent field visits and in-office documentation sessions to ensure compliant and current patient plans.

The ideal candidate holds an active NYS RN license, is able to travel within the assigned region, and can coordinate care with the Director of Patient Services.

Qualifications

• Current NYS RN license and registration in good standing
• Minimum 1 year of RN experience; home care or community health preferred
• Current BLS/CPR certification
• Ability to travel independently to patient homes in the assigned region
• Strong assessment, documentation, and communication skills

Responsibilities

• Conduct SOC comprehensive assessment in the home: physical, cognitive, functional, psychosocial, nutritional, medication, and home safety
• Develop an individualized plan of care consistent with physician/NP orders
• Reassess and update the plan of care with any change in condition or six‑month intervals
• Verify current medical orders are in place for all services; obtain or clarify orders as needed
• Educate patients and families on the plan of care, safety, infection control and when to contact the agency
