FreedomCare in New York, NY is seeking a Field Nurse to join our licensed home care team. You will assess patients, develop care plans, and supervise caregivers in patients’ homes, ensuring high-quality, compliant care.

The role requires a BSN or associative nursing credential with NYS licensure, 1–2 years in home care, strong communication, and the ability to travel across the five boroughs. We offer competitive pay, benefits, and growth opportunities.

Qualifikationen

Bachelor’s degree in nursing with NYS license (or compact) in good standing.
1–2 years nursing experience in home care.
Ability to travel within NYC boroughs and work field-based.
Strong communication and documentation skills.

Aufgaben

Conduct initial assessments of clients to determine care needs and develop individualized care plans.
Train and supervise non-skilled caregivers in providing personal care and daily living activities.
Monitor clients’ well-being and report changes to healthcare professionals.
Educate clients and families on effective care strategies and safety.
Ensure compliance with agency policies and state regulations.
Maintain accurate documentation of client care and progress.
