Job summary

CBH Care is seeking a dedicated professional in Morris Plains, NJ for per diem on-site case management. The role focuses on implementing IRPs, performing IMR services, and contributing to IRP updates within a multidisciplinary behavioral health team.

The position requires a BSW or related degree, a valid driver’s license, and the ability to document consumer data accurately using electronic charts. Flexibility with scheduling and opportunities for growth are offered.

• Provides on-site case managements services to assigned caseload and supervision to consumers
• Facilitates a variety of social, recreational, and therapeutic groups, encourages consumers to participate
• Utilizes crisis intervention skills as needed and reports all consumer concerns to program managers
• Must be able to follow our agency’s documentation procedures correctly when updating our consumers’ electronic charts
