Job summary

Select Medical's Case Manager (PRN) role at the Select Specialty Hospital LTACH requires current licensure in nursing, RT, or social work per state guidelines. Prior discharge planning and inpatient experience are highly preferred as you support complex, chronically ill patients through care coordination.

You will perform utilization reviews, discharge planning, and documentation, reporting to the Director of Case Management.

Qualifications

• Current licensure in a clinical discipline (RN, LPN, LVN, RT) or Social Work per state guidelines.
• Previous discharge planning experience preferred.
• Experience in inpatient hospital settings with critical/acute care patients.

Responsibilities

• Utilization reviews and resource management.
• Discharge planning and treatment plan management.
• Financial management and medical record documentation.
• Report to the Director of Case Management and provide social work services as needed.
• Coordinate multidisciplinary meetings and reimbursements with payors.
