Qualifications

Responsibilities

• Performs bio-psychosocial evaluations to plan treatment and discharge.
• Develops treatment plans and provides clinical intervention using individual and group modalities.
• Documents social work activity in medical records per departmental standards.
• Provides education to patients and families to restore optimum functioning.
• Supervises field placements for BSW/MSW candidates.
• Participates in conferences, care rounds, and interprofessional teams to coordinate care.
