• EXPERIENCE IN CLINICAL SOCIAL WORK PRACTICE AND ABILITY TO ADDRESS AGE, DEVELOPMENT, AND CULTURAL PATTERNS.

Responsibilities

• Conduct bio-psychosocial evaluations to identify barriers to treatment, discharge planning, and resource utilization.
• Develop and initiate treatment plans; apply individual and group modalities; monitor progress and adjust as needed.
