Qualifications

Responsibilities

• Completes bio-psychosocial evaluations to identify barriers to treatment, discharge, and care coordination.
• Develops treatment plans and initiates clinical intervention with individuals and groups.
• Documents social work activity in medical records per department standards.
• Participates in case conferences to improve care coordination across disciplines.
• Provides field placement supervision and evaluation for MSW/BSW students (LICSW or 5+ years experience required).
