Job summary

Fremont Hospital is hiring a Per Diem Clinical Case Manager to support patients with bio-psycho-social needs in a fully accredited psychiatric hospital. You will work with a physician-led treatment team to coordinate care, assess patients, and provide group therapies for patients and families.

The role requires a Bachelor’s degree in a relevant field, with a Master’s preferred, and at least 2 years of direct clinical experience in psychiatric settings.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Nursing, Social Work or related field required.
• Master’s degree preferred.
• Minimum 2 years direct clinical experience in psychiatric/mental health settings expected.
• Licensure: Master’s level pursuing licensure (MFT or ACSW) preferred in CA.

Responsibilities

• Provide clinical services to help patients meet bio-psycho-social needs.
• Assist patients in achieving emotional health through therapeutic treatment skills.
• Manage treatment activities: level system, treatment plans, psycho-social forms, and physician team interface.
• Provide psychotherapeutic groups and activities to patients and families.
• Oversee and coordinate legal paperwork and LPS regulations.
