Job summary

Universal Health Services' Fremont Hospital is seeking a Per Diem Clinical Case Manager to support inpatient psychiatric care. The role focuses on clinical services to address bio-psycho-social needs, treatment planning, and coordination with a physician-led team.

Qualified candidates will have a Bachelor’s degree in a relevant field, and a Master’s degree is preferred. Minimum 2 years of direct clinical experience in psychiatry/mental health is expected, with licensure pursuit possible in

Qualifications

• Bachelor’s degree in Social Services, Psychology, Nursing, Social Work, or Mental Health related field required.
• Master’s Degree preferred.
• Minimum 2 years direct clinical experience in a psychiatric or mental health setting and experience in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable entities is preferred.
• Masters level may be actively pursuing and registered for professional licensure by The California Board of Behavioral Sciences preferred (MFTi or ACSW).

Responsibilities

• Provide clinical services to assist patients in meeting bio-psycho-social needs.
• Coordinate treatment activities including completing psycho-social forms and entering treatment plan information.
• Interface with the physician-led treatment team and provide psychotherapeutic groups and activities.
• Oversee and coordinate legal paperwork and LPS regulations.
• Support inpatient psychiatric care across Fremont Hospital.

Education

Bachelor’s degree in Social Services, Psychology, Nursing, Social Work, or Mental Health related field
