Constellation Health Services invites applications for a Certified Home Health Aide (CHHA) in East Orange, NJ. The CHHA provides in-home care under the supervision of a nurse or therapist, observing and documenting patient conditions, and ensuring comfort and safety.

Must have a driver's license and vehicle to reach patients efficiently. Responsibilities include assisting with personal care, meals, light housekeeping, and reporting changes to the Case Manager.

Qualifications

• Must have a driver's license and a vehicle.
• Completion of at least 75 hours of classroom and supervised training for CHHA.
• High school diploma or GED.
• 18 years of age or older.
• Ability to read and follow written instructions and document care.
• Reliable transportation and willingness to commute to provide in-home patient care.
• Meets state training requirements, including orientation and competency evaluation.

Responsibilities

• Perform personal care tasks after given a patient's assignment and promptly attend to requests.
• Provide patient and family with positive communication within ethical and respectful care, including confidentiality.
• Meet safety needs of patient and use equipment safely.
• Provide baths and personal care (sponge, tub or shower), hair, skin, nail care as directed.
• Light housekeeping to maintain a safe environment (changing bed linen, light cleaning, laundering).
• Assist in dressing and undressing patients as assigned.
• Plan and prepare nutritious meals and assist in feeding as assigned.
• Assess and report on patient condition and significant changes to the Case Manager.

Constellation Health Services is seeking a Certified Home Health Aide (CHHA) to join our team in East Orange, NJ!The Home Health Aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse or licensed therapist and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The specific personal care for each patient will be determined by the registered nurse or therapist and will be given in writing to the certified home health aide - CHHA. The CHHA is responsible for observing the patient, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.MUST HAVE A DRIVER'S LICENSE AND A VEHICLE.Responsibilities:Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 ,etc.)Giving personal care including baths, (sponge, tub or shower) back rubs, care of hair, skin, oral hygiene, shampoos if directed by the registered nurse.Cleaning of nails and use of emery board. No cutting or clipping of nails is permitted.Performs light housekeeping duties necessary to maintain a safe environment including changing bed linen, light cleaning of patient area, and laundering essential to the comfort and cleanliness of the patient.Assisting in dressing and undressing patients as assigned.Planning and preparing nutritious meals, including shopping, as assigned.Assisting in feeding the patient as assigned.Assessing and reporting on patient's condition and significant changes to the Case Manager.
