Constellation Health Services in Jersey City, NJ is seeking a Certified Home Health Aide (CHHA) to support patients at home under the supervision of a nurse or therapist. The CHHA observes, reports, and assists with personal care to promote comfort and safety.

Must have a valid driver’s license and vehicle, complete at least 75 hours of classroom and supervised training with 10 hours orientation, and a high school diploma or GED.

Qualifications

• Must have a valid driver’s license and a vehicle.
• Complete a minimum of 75 hours of classroom and supervised training.
• Orientation: ten hours of orientation and a competency evaluation before providing patient care.
• High school diploma or GED required.
• At least 18 years of age.
• Ability to read and follow written instructions and document care.
• Understands home care philosophies and can provide specialized care as needed.
• Reliable transportation and willingness to commute for in-home care.

Responsibilities

• Perform personal care tasks as assigned and promptly attend to patient requests.
• Observe and report patient conditions to the Case Manager.
• Communicate with patients and families with compassion and confidentiality.
• Assist with dressing, bathing, grooming and daily activities.
• Plan and help prepare nutritious meals and assist with feeding as needed.
• Ensure safety and proper use of equipment and maintain a clean patient area.
