Job summary

Constellation Health Services in Sewell, NJ is hiring a Certified Home Health Aide (CHHA) to support patients in their homes under RN or therapist supervision. The CHHA will provide personal care, observe and report patient conditions, and document care provided, while promoting safety and comfort.

Applicants must have a NJ CHHA certification, a valid driver's license and reliable transportation, and complete required training and orientation. High school diploma or GED is required; 18+.

Responsibilities

• Perform personal care tasks as assigned and respond promptly to patient requests.

• Assist with dressing, meals, feeding, and light housekeeping as needed.
• Observe, report, and document changes in patient condition to Case Manager.
• Ensure safety and proper use of equipment; maintain confidentiality.

Constellation Health Services is seeking a Certified Home Health Aide (CHHA) to join our team in Sewell, NJ!The Home Health Aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse or licensed therapist and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The specific personal care for each patient will be determined by the registered nurse or therapist and will be given in writing to the certified home health aide - CHHA. The CHHA is responsible for observing the patient, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.MUST HAVE A DRIVER'S LICENSE AND A VEHICLE.Responsibilities:Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 ,etc.)Giving personal care including baths, (sponge, tub or shower) back rubs, care of hair, skin, oral hygiene, shampoos if directed by the registered nurse.Cleaning of nails and use of emery board. No cutting or clipping of nails is permitted.Performs light housekeeping duties necessary to maintain a safe environment including changing bed linen, light cleaning of patient area, and laundering essential to the comfort and cleanliness of the patient.Assisting in dressing and undressing patients as assigned.Planning and preparing nutritious meals, including shopping, as assigned.Assisting in feeding the patient as assigned.Assessing and reporting on patient's condition and significant changes to the Case Manager.
