Qualifications

Responsibilities

• Conduct intake interviews and determine needs for medical and psychosocial evaluations.

• Advocate for patients and arrange post-inpatient services.
• Communicate findings and plans with physicians, therapists, and other care team members.
• Support timely follow-up and resource coordination for post-discharge care.
