Job summary

Mass General Brigham's Spaulding Rehabilitation Hospital-Cambridge is seeking a full-time case manager to support intake interviews, evaluate medical and psychosocial needs, and coordinate post-discharge plans with the clinical team.

Ideal candidates will have case management or discharge planning experience, LTAC exposure, and an MA license (RN, PT, OT, SLP, RT or LCSW/LICSW). The role requires strong communication, advocacy, and collaboration with patients, families, and providers.

Qualifications

• Experience in case management, utilization review, or discharge planning is preferred.
• LTAC experience is preferred to coordinate post-acute care.

Responsibilities

• Conduct intake interviews and determine medical and psychosocial needs.
• Coordinate discharge and aftercare services with the clinical team.
• Advocate for needed services and entitlements for patients.
