Spaulding Nursing and Therapy Center Brighton, Inc. is seeking a Per Diem Case Manager to perform intake interviews, evaluate medical and psycho-social needs, and coordinate discharge planning and post-discharge services.

This onsite role requires active collaboration with physicians, therapists, and the care team to ensure timely and appropriate care planning. The ideal candidate holds a Bachelor's in a related field and licenses such as RN, PT, OT, RT or LCSW/LICSW, with 2–3 years of case

Qualifications

  • Bachelor's Degree in related field required.
  • 2–3 years of case management, utilization review, or discharge planning experience preferred.
  • RN, PT, OT, RT or LCSW/LICSW licensure required.
  • Strong customer service and interpersonal skills.
  • Knowledge of community resources and ability to coordinate services.

Responsibilities

  • Discuss assessment findings with physician or treatment team to maximize outcomes.
  • Assist in discharge planning and coordination of aftercare services.
  • Communicate effectively with internal and external teams.
  • Coordinate discharge and post-acute care with clinical guidance.
  • Support the team and follow through on requests and timelines.
  • Initiate comprehensive assessments to identify baseline data and potential barriers.
  • Coordinate resources post-discharge for homecare, outpatient, or alternate care.
  • Evaluate ongoing appropriateness of facility level of care with payer collaboration.
  • Comply with State and Federal regulations and credentialing bodies.

Yes Essential Functions -Discusses assessment findings and patient status with physician and /or treatment team to maximize outcomes. -Assist in discharge planning as required -Communicating effectively and efficiently internally and externally. -Coordinates discharge and after care services with guidance from the clinical team -Assists others and supports the team. -Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner. -Initiate a complete assessment, to gather and/or verify relevant baseline data, identify their understanding and expectations of their stay, and identify any potential issues/barriers to arranging post inpatient care. -Proactively work with patient, caregiver, and the interdisciplinary team, including community providers of medical services, as appropriate, to develop a realistic and timely plan for post inpatient care. -Identify and coordinate appropriate resources post discharge for patients requiring homecare, outpatient, or placement in an alternate level of care. -Perform ongoing evaluation for appropriateness for the facility’s level of care by applying knowledge of level of care criteria with active collaboration with patients insurance payer -Comply with all applicable State and Federal regulations as well as other credentialing bodies, including but not limited to, Joint Commission and CARF, regarding the case management process.**Qualifications**Education Bachelor's Degree Related Field of Study required Can this role accept experience in lieu of a degree?
