Job summary

Spaulding Rehabilitation Hospital Cambridge is seeking a Case Manager (full-time, day shift) to coordinate intake interviews, evaluate medical and psycho-social needs, and plan post-discharge services. The role requires MA licensure (RN, PT, OT, SLP, RT or LCSW/LICSW) and 2–3 years of related experience.

A sign-on bonus up to $10,000 is offered. Responsibilities include communicating with physicians and care teams, advocating for services, and ensuring timely access to entitlements.

Qualifications

• Bachelor's Degree related field required.
• 2-3 years of case management, utilization review, or discharge planning preferred.
• Strong customer and interpersonal skills; proficient with Microsoft Office.

Responsibilities

• Discuss assessment findings with physician/treatment team to maximize outcomes.
• Assist in discharge planning and coordinate discharge/aftercare services.
• Communicate effectively with internal/external teams and patients.
• Identify resources post-discharge and coordinate post-inpatient care.
• Perform ongoing evaluation to ensure appropriate level of care and adherence to regulations.
