Job summary

The Dimock Center in Boston, MA is seeking a Per Diem Case Manager for Clinical Stabilization Services to provide care coordination and discharge planning for residents in substance use treatment. You will collaborate with clinicians, medical providers, and community partners to help clients access services and transition back into the community.

The role requires at least one year in a substance use treatment setting, a Bachelor's in a related field, and strong communication skills.

Qualifications

• Bachelor's degree in Human Services, Psychology, Sociology, Health Services, or related field.
• Master's degree or current enrollment in MSW program preferred but not required.
• At least one year of experience in a substance use treatment program.
• Strong written and verbal communication skills.
• Commitment to confidentiality and professionalism.

Responsibilities

• Meet regularly with clients to provide care coordination and aftercare planning.
• Conduct assessments to identify client needs, strengths, and barriers to treatment goals.
• Develop care plans in collaboration with clients and the treatment team.
• Coordinate referrals to housing, employment, healthcare, and financial supports.
• Coordinate care with courts, primary care providers, and behavioral health specialists.
• Facilitate psychoeducational groups as assigned.
• Provide transportation to appointments or community services when needed.
• Plan and coordinate discharge and aftercare services to ensure continuity of care.
• Conduct post-discharge follow-up with clients.
• Assist the clinical team with crisis management as needed.
• Complete documentation in a timely manner per Joint Commission standards.
• Attend required staff meetings and trainings.
