Montefiore St. Luke's Cornwall is seeking a dedicated Case Manager for a Saturday per diem role. You will coordinate patient care across hospitalization, guiding interdisciplinary plans to meet treatment and discharge needs and to achieve optimal clinical and resource outcomes.

The ideal candidate holds a BSN or MSW with current New York licensure, 1–3 years acute care experience, and relevant certifications (CCM, LCSW, PRI). This per diem position offers houry pay and variable daily hours.

Qualifications

• BSN graduate from NLN-accredited school of nursing required.
• MS in Social Work from an accredited school required.
• 1-3 years experience in an acute care environment required.
• Ability to read, write, and communicate in English.
• Strong clinical, organizational, and interpersonal skills.
• Registered Nurse New York - New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse. Required
• Licensed Clinical Social Worker - New York State Board of Social Work within 180 Days Required
• CCM - Certified Case Manager - Commission for Case Manager Certification Certification in Case Management required within 1-1/2 Yrs Required
• PRI - Patient Review Instrument Cert - PHTC PRI certification Preferred

Responsibilities

• Coordinates the care and services of patients across the continuum of their hospitalization.
• The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs.
• This role aims to achieve optimal clinical and resource outcomes.
• Collaborates with the interdisciplinary team to plan and facilitate timely discharge.
• Performs duties as a Saturday per diem position with flexible daily hours.
