Job summary

Main Line Health is seeking a Care Coordination Registered Nurse (RN) at Lankenau Medical Center. You will orchestrate patient care across the continuum, collaborating with physicians and families to implement and monitor a comprehensive care plan focused on health promotion, disease prevention, and management.

Per diem position with mainly weekends; 16 hours/month plus one weekend per month and one holiday per year. A BSN is preferred and a BSN within 3 years if you hold an Associate Degree.

Qualifications

• Three years of recent clinical experience required.
• Leadership experience preferred.

Responsibilities

• Coordinate and oversee patient care across the continuum.
• Partner with physicians, families, and colleagues to develop and monitor a comprehensive plan of care.
• Engage in hospital nursing committees and system-wide projects.

Shift:

Experience:
