Job summary

Main Line Health is seeking a Care Coordinator RN at Bryn Mawr Hospital to partner with physicians, families, and multidisciplinary teams to orchestrate care across the continuum, enhancing patient experiences and outcomes.

You will develop and monitor a comprehensive plan of care, emphasizing health promotion, disease prevention, and disease management, while leading smooth care transitions and collaborating on system-wide improvements.

Qualifications

• 3 years of recent clinical experience required; leadership experience preferred.
• If ADN, 10 years of experience required.
• Associates Degree or BSN required; if associates, BSN within 3 years of hire.
• Active PA RN license required.

Responsibilities

• Coordinate care across the continuum with physicians, peers, and community partners.
• Develop and monitor a comprehensive plan of care, including health promotion and disease management.
• Lead care transitions and strive to improve patient experiences and outcomes.
