Position Overview

Registered Nurse RN - Care Coordination

Responsibilities

You will orchestrate your patients' care across the healthcare continuum, working with physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care experiences and services, improve population health, reduce cost, and achieve collaborative practices. You will use communication and advocacy skills to partner with patients, families, physicians, and colleagues to develop, implement, and monitor a comprehensive plan of care that includes health promotion, disease prevention, and disease management activities.

Qualifications

• Three years of recent clinical experience required
• Leadership experience preferred
• Associate’s Degree or BSN required upon hire; if hired with an Associate’s Degree, a BSN must be obtained within 3 years of hire
• Master’s Degree preferred
• Active PA RN license required

Position Details

• Shift: Per Diem, Varied Shifts; mainly weekends, some weekdays to cover. 16 hours/month + one weekend per month + 1 holiday per year
• Schedule: Per Diem
• Employee Status: Regular
• Pay Range: $46.00 - $46.00
