The Care Team Coordinator (CTC) is responsible for the daily operations of the patient care team within HomeCare & Hospice. Individual is responsible for the coordination and scheduling of all clinical care across the disciplines and health care providers. The CTC coordinates the day, evening and weekend operations of the care team. The Care Team Coordinator works in conjunction with the Division Manager to develop and monitor patient care services based on customer needs. The Care Team Coordinator is responsible for clinical outcomes and patient satisfaction for designated team(s). The CTC provides leadership within the clinical team and facilitates the provision of high-quality patient care and services. They participate in performance improvement initiatives of the organization. The Care Team Coordinator oversees new employee orientation, on-going staff competencies, and assists with 90 day and annual performance reviews. The CTC is accountable for employee engagement for designated team(s).
