Main Line Health is seeking a Care Team Coordinator for HomeCare & Hospice in Radnor, PA.

The CTC leads daily operations, coordinates clinical care across disciplines, and manages day/evening/weekend coverage while driving quality and patient satisfaction.

Requirements include RN with PA license, BSN, CPR, and a driver’s license; five years clinical experience and three years in home care/hospice are preferred.

Qualifications

• Five years clinical experience required.
• Three years’ experience in the home care or hospice setting preferred.
• Registered Nurse, BSN required; MSN or equivalent preferred.
• Licensed in Commonwealth of PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
• CPR Certification required.
• Current Driver’s license and proof of auto insurance required.
• Specialty nursing certification preferred.

Responsibilities

• Oversee daily operations of the patient care team and coordinate scheduling across disciplines.
• Manage day, evening and weekend operations for the care team.
• Collaborate with the Division Manager to develop and monitor patient care services.
• Contribute to performance improvement initiatives and ensure patient satisfaction.
• Oversee new employee orientation, ongoing staff competencies, and conduct 90 day and annual performance reviews.
• Promote employee engagement within designated teams.

The Care Team Coordinator (CTC) is responsible for the daily operations of the patient care team within HomeCare & Hospice. Individual is responsible for the coordination and scheduling of all clinical care across the disciplines and health care providers. The CTC coordinates the day, evening and weekend operations of the care team. The Care Team Coordinator works in conjunction with the Division Manager to develop and monitor patient care services based on customer needs. The Care Team Coordinator is responsible for clinical outcomes and patient satisfaction for designated team(s). The CTC provides leadership within the clinical team and facilitates the provision of high-quality patient care and services. They participate in performance improvement initiatives of the organization. The Care Team Coordinator oversees new employee orientation, on-going staff competencies, and assists with 90 day and annual performance reviews. The CTC is accountable for employee engagement for designated team(s).

Day shift (8:30 am to 5:00 pm); Monday - Friday

Experience:

• 1. Five years clinical experience required.
• 2. Three years’ experience in the home care or hospice setting preferred.

Education:
