Qualifications

• New Grad or RN with less than 12 months experience.
• Per diem outpatient ambulatory care environment experience preferred.

Responsibilities

• Provide ambulatory care and preventive services to patients.
• Educate patients and coordinate care with physicians and staff.
• Support continuity of care across multiple visits and teams.
• Collaborate with the care team in an outpatient setting.
