North Shore Community Health, Inc. seeks a Per Diem Advanced Practice Provider (NP or PA) to join the Primary Care Team across multiple sites in Gloucester, Salem, and Peabody.

You will provide direct care, conduct exams, perform history taking, and use diagnostics in ambulatory settings, including telemedicine. Applicants must be nationally certified (FNP or AGNP for NP; MA licensure for PA) and hold Mass DPH CS/DEA registrations.

Competenze

• NP: national certification (FNP or AGNP) and state APRN licensure in MA
• PA: national certification and MA licensure to practice as a Physician Assistant
• DPH CS registration and DEA registration required (Massachusetts)

Mansioni

• Provide direct care to scheduled and walk-in patients, including exams and treatments.
• Conduct history-taking and physical exams; use diagnostics to inform treatment plans.
• Conduct visits in person and via telemedicine across Gloucester, Salem, and Peabody; refer to specialists as needed.
• Maintain patient confidentiality per HIPAA and NSCH policies.
• Collaborate with the Primary Care Team to support quality initiatives and patient care.
