Job summary

Creative Foundations, Inc. is seeking a Supportive Living Coordinator (SLC) to lead and supervise Direct Support Professionals delivering services to individuals with intellectual and developmental disabilities.

You will supervise staff, schedule coverage, develop ISPs in partnership with families, and ensure compliance with Ohio requirements and agency policies. In this role, you will coordinate with families and stakeholders, provide training, document outcomes, and promote person-centered

Qualifications

• High school diploma or GED is required.
• Experience supporting individuals with I/DD preferred.
• Supervisory or team lead experience preferred.
• Experience with Direct Support Professionals or community-based services is a plus.

Responsibilities

• Train, supervise, and support Direct Support Professionals.
• Create and maintain weekly staff schedules.
• Develop and monitor Individual Service Plans (ISPs).
• Coordinate with families and stakeholders for service delivery.
• Ensure compliance with DODD, Medicaid, and agency policies.
• Lead monthly team meetings and document performance and incidents.
