Qualifications

• Bachelor's degree in Physical Therapy is required.
• Applicable state Physical Therapy license is required.

Responsibilities

• Evaluates patients and selects assessment techniques to establish care plans.
• Interprets data from medical records, physician reports, patients, and families.
• Establishes functional goals with patients and provides evidence-based interventions.
• Documents evaluations, progress notes, discharge notes, and charges as needed.
• Communicates progress to physicians, referral sources, and clinical staff; coordinates schedules and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
