Job summary

Advocate Health is seeking a Physical Therapist to rotate between Morehead Medical Plaza, Arboretum, and Ballantyne locations. The role requires Pelvic Physical Therapy experience, a current state license, and BLS certification.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Applicable state Physical Therapy license is required.
• BLS certification required per policy.

Responsibilities

• Evaluates patients and develops individualized care plans with modalities and exercises.
• Gathers data from medical records and families for accurate documentation.
• Sets functional goals with patients and adjusts plans as needed.
• Documents evaluations, progress notes, and charges as required.
• Communicates progress to physicians and staff; coordinates schedules and discharge planning.
• Provides supervision of PTAs and Rehab Tech/Aide as required for licensure.
