Job summary

IMind Health is seeking a Community Support Worker for PRP/MTS programs in Lanham, MD. This contract role focuses on delivering community-based support, case management, and rehabilitation services to individuals living with severe mental illness.

The successful candidate will work in community settings and participants’ homes when appropriate, maintaining professional boundaries, and adhering to safety protocols. Excellent engagement and organizational skills are essential.

Qualifications

• Experience providing case management, direct support, or behavioral health services to individuals with severe and chronic mental illness.
• Experience with PRP and/or MTS-like services is strongly preferred.
• Strong interpersonal, verbal, and written communication skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Provide direct support, case management, and rehabilitation services to program participants.
• Develop and maintain professional relationships with participants to build trust.
• Assist participants with identifying needs, setting goals, and connecting to resources.
• Transport participants to appointments and community activities as appropriate.
• Assist with accessing social services, housing resources, healthcare, transportation, and other supports.
• Provide services in community settings and in participants' homes when appropriate and safe.
