Job summary

Montefiore Mount Vernon Hospital is seeking a Peer Specialist who will educate clients on coping strategies, share experiential insights, and help set rehabilitation goals under supervision of the Program Manager.

You will use your lived experience to support health and wellness, assist with community integration, coordinate referrals, and maintain accurate records while participating in trainings and rounds throughout the ACT program.

Qualifications

• Must have NYS provisional or permanent certification as a peer specialist.
• High School Diploma or GED equivalent required.
• Associate or bachelor's degree preferred.
• Must have a clean NYS driver's license.
• Good verbal, writing and computer skills.
• Bilingual (Spanish/English) preferred.

Responsibilities

• Serve as role model and mentor to clients.
• Assist clients in developing community support networks.
• Coordinate services, monitoring and follow-up.
• Refer to community providers (DSS, Housing, DCMH, Legal, Financial).
• Advocate for navigating social services to obtain entitlements.
• Promote self-help and independent living.
