Job summary

Montefiore Mount Vernon Hospital is seeking a Peer Specialist to educate clients on coping strategies and recovery goals, drawing on personal experience to support wellness and community linkage.

Under the Program Manager, you will mentor clients, coordinate services, advocate for entitlements, and help build durable community networks while maintaining accurate records and participating in core trainings.

Qualifications

• Must have NYS provisional or permanent certification as a peer specialist.
• High School Diploma or GED equivalent required.
• Associate or bachelor’s degree preferred.
• Must have a clean current NYS driver’s license.

Responsibilities

• Serve as role model and mentor to clients.
• Assist clients in developing community support systems and networks.
• Service coordination, monitoring and follow-up.
• Coordinating referrals to community/service providers (DSS, Housing, DCMH, Legal, Financial).
• Provide advocacy and assistance in navigating the social services system to obtain entitlements.
• Promote and support self-help and independent living.
• Escort clients to appointments and interviews when necessary.
• Act as liaison with community agencies, hospitals for admission and discharge planning.
• Participate in morning rounds, case conferences, staff meetings and training programs.
• Maintain up-to-date records as required by program procedures and regulatory agency requirements.
• Attend and complete ACT Core Trainings as required by NYSOMH.
