Job summary

Qualifications

Responsibilities

• Build meaningful connections and provide mentorship throughout recovery.
• Manage a caseload, identify strengths, set personalized recovery goals, and monitor progress.
• Connect clients to community resources including housing, Medicaid, and employment services.
• Facilitate site-based peer support groups and individual outreach.
• Visit and transport clients as needed per program guidelines.
• Coordinate with families, case managers, and providers to promote continuity of care.
• Maintain regular contact with clients to support progress and address barriers.
• Complete timely documentation in the agency’s systems.
