• Coordinate discharge planning and linkage to treatment.
• Support patients in their recovery using motivational interviewing techniques.
• Document services in the medical record and collaborate with the care team.
• Participate in data reporting to state agencies.
• Work in a safety-sensitive role requiring alertness.
