Job summary

Qualifications

• High School Diploma or equivalent required.
• Experience with behavioral health or recovery services preferred.
• Knowledge of care transitions and Zero Suicide model is a plus.

Responsibilities

• Provides peer recovery services to individuals at risk for substance misuse or misuse disorders.
• Engages patients using Motivational Interviewing to build rapport and trust.
• Connects patients to community resources and coordinates referrals for treatment.
• Maintains timely and accurate documentation in the EHR and records patient outcomes.
• Supports care transitions and follow-up after discharge with treatment providers.
• Runs recovery groups in ambulatory and inpatient settings.
