Qualifications

Responsibilities

• Build supportive, trusting relationships with individuals in recovery.
• Manage an assigned caseload and identify recovery goals and plans.
• Connect clients to resources including housing, Medicaid, employment, and services.
• Facilitate daily peer groups and community-based support.
• Maintain documentation in accordance with requirements.
• Utilize EHR and other systems to record client information.
