Zusammenfassung

Cascadia Health is seeking a Mobile Peer Provider to deliver trauma-informed, strength-based services in the community, in coordination with clinicians in their area. The role emphasizes respect, mutuality, and peer-based support to clients facing mental health or developmental challenges.

The Mobile Peer Provider will collaborate with mental health and physical health teams, law enforcement, schools, and social services to ensure optimal client care and outcomes.

Qualifikationen

• Must hold Oregon Health Authority-approved Peer Support credentials (PSS or PWS).
• THW certification required.
• Experience with behavioral health and diverse communities preferred.

Aufgaben

• Provide immediate mobile response to individuals experiencing mental health crises in Multnomah County.
• Provide mobile support to individuals or families facing emotional, developmental, or health challenges.
• Share lived experience to inspire hope and model positive outcomes for individuals or families.
• Assist in developing and implementing communication, crisis, and safety plans.
• Help access local resources such as housing, public benefits, education, and health services.
• Mentor skills related to parenting, positive communication, and self-care.
• Assist in treatment team planning, safety planning, and policy meetings.
• Assist in completing applications and forms related to services.
• Provide culturally responsive services, particularly for historically underserved populations.
