Job summary

VIP Community Services is seeking a Peer Navigator / Data Entry Associate in New York to provide peer-based support and concrete services for individuals with medical, behavioral health, mental health, and substance use needs. You will document services in AIRS and eClinicalWorks, ensuring accuracy and timely reporting, while maintaining HIPAA compliance and professional boundaries.

The role includes clinical adherence management, outreach, appointment reminders, and collaboration with MSA

Qualifications

• HS Diploma or equivalent is required.
• Experience with the target population is preferred.
• Excellent written and verbal communication skills are essential.
• Ability to read, write and speak English.
• Bilingual English/Spanish is a plus.

Responsibilities

• Provide clinical adherence management to clients per agency policies and regulations.
• Conduct outreach to clients facing adherence challenges in medical, behavioral health, HIV care.
• Make reminder calls and follow-up for missed appointments.
• Assist re-engagement of clients lost to care.
• Accompany clients to appointments and support groups.
• Facilitate peer support groups and adherence assistance.
• Document client interactions accurately in ECW and AIRS.
• Attend meetings, trainings, and special projects as assigned.
• Maintain HIPAA compliance and professional boundaries.
• Maintain organized electronic records and meet reporting timelines.
