Job summary

VIP Community Services seeks a Peer Navigator / Data Entry Associate to support clients with medical and behavioral health needs in New York. The role centers on peer-based engagement and precise data entry to ensure care continuity.

The position requires experience with the target population, strong English communication skills, and familiarity with eClinicalWorks and AIRS. Bilingual English/Spanish is a plus; in‑person multi-program duties are expected.

Qualifications

• HS Diploma or equivalent.
• Experience working with the target population is required.
• Excellent written and oral communication skills.
• Ability to read, write and speak English; bilingual English/Spanish a plus.

Responsibilities

• Provide peer-based support and services to clients with medical, behavioral health, HIV, and substance use needs.
• Maintain accurate, timely documentation in AIRS and eClinicalWorks.
• Conduct outreach and reminder calls to improve client engagement in care.
• Assist with re-engagement and accompany clients to appointments as appropriate.
• Document interactions and participate in trainings and meetings as assigned.
